You must have a signed & dated print-out* from the Financial Aid office! Print and Fill out application Grad Students must pay Community Activities fee! Bring your application & Financial Aid printout to KUB 428 No loans processed after 4:00pm!	Faculty, Staff, and Grad. Students Print and Fill out Community Activities Card Application. Bring into KUB 428. Make checks payable to "Community Activities". *Graduate Students Fee: $25 for One Semester $35 for Two Semesters	Student Organizations Print and Fill out Requisition *Bring into KUB 428	Production is in progress for the new Student Telephone Directory. Information in the directory is taken from data supplied by students to the Registrar. Students may withhold disclosure of the student information under the Family Educational Rights and Privacy Act of 1974. *Students who do not wish to have directory information published must file notice with the Office of the Registrar. *This must be done within the first ten days after the beginning of the semester. If students do not specifically request withholding directory information, it indicates approval for the disclosure.	Print and Fill out Rec. Center Application. Bring to KUB 428. *Make checks payable to "BU-SRC".
